Who are we?

  • Providing all types of care for all ages.
  • Fees charged are based on the patient’s income and ability to pay.
  • The fee scale is set by federal guidelines.
  • Offer more services than a general doctor’s office, just ask.
  • Our patients' opinions and needs matter, over half of our Board members are patients as well.

How can you get care here?

  • To receive medical care, call 367-8793 for an appointment
  • To use sliding scale payment, please bring a W-2 or pay stubs with you on your first appointment
  • If on Medicare with managed care bring proof of the doctor who has been assigned to you

The Clinics

  • Includes exam and procedure rooms, laboratory, and x-ray rooms.
  • dental operatories.
  • A large staff that works together to meet all your health needs.
  • Open to meet your needs, including evenings on Mondays and acute care on Saturday mornings (September through April).

Appointments

  • After your first visit, you will get a FCH ID card, bring it to each clinic visit.
  • Also bring it to the emergency room to get a discount on the physician’s bill.
  • Bring your Medicaid or Medicare card for clinic and emergency room visits as well.
  • For appointments, call ahead to reserve a time to see the doctor.
  • The number to call is 367-8793.
  • For acute problems needing same day care, call during our hours so we can help take care of you.
  • For emergencies go to the emergency room at the hospital.
  • If you have to cancel an appointment, please do so as soon as possible-preferably 24 hours ahead of time.
  • After hours, you can still see a doctor by calling the above number and leaving your name and number and they will call you back.
