Homeless Outreach

Taking health care outside the clinic walls.

Falls Community Health provides health care to people who are homeless by assisting them to arrange appointments in the clinic or by seeing them in the community. Staff is available at outreach sites in the community. They are available to provide patient education and support inside and outside the clinic.

Referral indicators for the homeless program include (but not limited to):

  • Complex health issues
  • ADL concerns
  • Need for specialty referrals
  • Case management
  • Coordination of care
  • Medication issues
  • Patient Education
  • Post-appointment follow up
  • Dental/Vision
  • Housing
  • Addictions/Dependency
  • Employment
  • Education
  • Finances
  • Food/Clothing
  • Legal

Other services available on a limited basis:

  • Dental Clinic voucher
  • Transportation
  • Home Visits
